Home & Community Care

This program delivers the services of Elder Support and Home Care from Long Term Care Aids and is supported by two registered nurses. The duties of the Long Term Care Aids are to provide care and assistance in Elder’s homes and work closely with the nurses at the Health Centre to help them provide adequate care for our Elders who have health problems or who have just had surgery. This program provides the services of home management, meal preparation, transportation and personal care assistance. The Elder Support Worker provides support to Elders who are not able to do tasks or who have difficulty in doing tasks for themselves. This also includes picking up and dropping off medications and giving rides to Elders who need assistance.

The nurses provide medical service to our Elders and keep in close contact with the
doctors regarding conditions of patients or concerns that may arise. They do dressing changes, remove stitches and help with dispensing medication when necessary. They also monitor vital signs, and deal with wound management and crisis intervention.

The focus of our Home and Community Care program is to keep our Elders in their
own homes as long as possible; promoting ease and health in their lives to keep them healthier and able to care for themselves longer.
